Form 300: the log
OSHA Form 300 is the log used to record qualifying work-related injuries and illnesses for covered employers. Entries are maintained as recordable cases occur during the year.
Form 300A: the summary
Form 300A summarizes the establishment’s injury and illness information for the year. OSHA has separate posting and, for some establishments, electronic submission requirements.
Form 301: the incident report
Form 301 provides more detail about an individual injury or illness incident. OSHA permits equivalent forms in certain circumstances when they contain the required information.
Who needs the forms
Not every employer is required to keep these records. OSHA’s recordkeeping rules include size and industry exemptions, so employers should verify the requirements that apply to their establishment.
