Official CMS-1500 specification sources
The National Uniform Claim Committee (NUCC) maintains the 1500 Claim Form and its reference instruction manual. CMS publishes Medicare-specific paper-claim guidance and points users to Chapter 26 of the Medicare Claims Processing Manual for completion and printing requirements.
Version 02/12
The current form version is 02/12. NUCC states that use of the 02/12 form went into effect April 1, 2014, and its current reference instruction manual for the 02/12 form was released in July 2025.
OCR-red paper submission
CMS states that acceptable CMS-1500 submission forms use Flint OCR Red, J6983, or an exact match. The form is commonly processed with optical character recognition, so form color, scale and alignment matter.
Practical printing checks
- Confirm the payer accepts paper claims for the claim you are filing.
- Use compliant CMS-1500 form stock when required.
- Keep the printed claim data inside the intended boxes.
- Do not let printer scaling unexpectedly shrink or enlarge the output.
- Test X/Y alignment before printing a batch.
- Follow the payer's current instructions when they differ from general guidance.
What changed from the older form?
NUCC publishes a 02/12 change log showing changes from the older 08/05 version. The 02/12 form was designed in part to align the paper claim with information used in the electronic professional claim workflow.
