CMS-1500 printer alignment guide
Use this checklist to reduce shifted or scaled printing on preprinted CMS-1500 forms.
Before changing alignment
- Set the printer to Actual size or 100%.
- Turn off Fit to page, Shrink, or Scale to fit.
- Use the correct paper size and portrait orientation.
- Load one blank test sheet before using an official red form.
Correct the position
If all fields print too high or low, adjust the vertical or top setting. If all fields print too far left or right, adjust the horizontal or left setting. Make small adjustments and print another test.
Common causes
- Printer driver scaling
- Browser or PDF viewer set to Fit
- Incorrect paper tray or paper size
- Margins added by the printer
- Using a photocopied form that is not the exact official size
Final check
Verify the patient, provider, diagnosis, service-line, and signature information stays within the intended boxes. Test with fictional information before printing a real claim.
