Use this workbook to import multiple CMS-1500 claims into the Paper and Electronic Filing Software. The format keeps each claim together while allowing Box 24 service lines to continue vertically instead of creating hundreds of columns.
CMS1500-Import-Sample.xlsx
This is the only sample workbook displayed on this page. It includes working sample claims and a Read Me tab with the import rules.
Download Excel Import TemplateThe workbook uses one Excel row for each Box 24 service line. Claims with multiple services simply continue on the rows below.
Open the Import Data tab. Keep the existing column names exactly as they appear.
Give each claim a unique Import Claim ID, such as CLAIM-001. Repeat that ID on every service-line row for the claim.
Use Service Line # values 1, 2, 3 and so on. Each row represents one Box 24 service line.
Save as .xlsx, then choose Bulk Claims > Import Excel / CSV. Review the Import Preview before anything is added.
These rules let one workbook handle simple one-line claims and larger professional claims without making the spreadsheet difficult to use.
Enter patient, insured, payer, diagnosis, billing provider, service facility and other claim-level fields on the first row of the claim. On rows 2 and higher, those claim-level cells can stay blank.
Dates of service, place of service, procedure code, modifiers, diagnosis pointers, charges, units and rendering-provider information belong on the applicable service-line row.
Import Claim ID tells the software which rows belong to one claim. Service Line # controls the order of the Box 24 lines.
The software maps the workbook by the supported column names. Keep the header row intact and enter your data underneath it.
The workbook contains three sample claims so you can see exactly how a multi-line claim is stacked.
The same workbook works for both workflows inside the combined CMS-1500 program.
Service lines 1–6 fit on one CMS-1500 claim form. Claims with additional lines require the paper continuation/split workflow.
The importer can retain service lines 1–50 for the electronic workflow, allowing larger professional claims to stay grouped under the same Import Claim ID.
The software groups the spreadsheet rows into claims first, then shows what will be imported.
Confirm the patient, Import Claim ID, original Excel row range and number of service lines for each grouped claim.
Missing Claim IDs, duplicate service-line numbers, invalid line numbers and conflicting repeated claim information are shown before import.
Use Errors Only to hide ready claims and focus only on claims and rows that need correction.
If needed, export the errors to CSV with the original Excel row numbers and messages so the workbook can be corrected quickly.
Use only the CMS1500-Import-Sample.xlsx workbook linked at the top of this page. Older sample files are no longer shown here.
Yes. Continue adding rows under the same Import Claim ID and increase Service Line # sequentially. The electronic 837P workflow can retain up to 50 lines.
No. Enter the claim-level information on Service Line 1. Continuation rows normally need only the Import Claim ID, Service Line # and the Box 24 service-line data.
The Import Preview identifies structural and claim-level problems before the records are added. Use the Errors Only filter to focus on the rows that need correction.
