A standard color printer is not a reliable way to create a submit-ready CMS-1500 red form. The background color, line placement, paper, and form dimensions must meet the specifications used by scanners and payers.

Recommended approach

For paper submission, purchase approved pre-printed CMS-1500 forms and print only the black claim information over the red form. Use alignment controls to keep characters inside the intended fields.

What a color printer can still do

A color or black-and-white copy can be useful for internal records, training, or review. Do not assume a copy produced on ordinary paper is acceptable for claim submission. Check the payer’s current paper-claim instructions.

Billing reminder: Payer and program requirements vary. Confirm the current instructions for the payer receiving the claim.
