A black-and-white CMS-1500 printed on ordinary paper is useful as a file copy, preview, or training document. It should not automatically be treated as a submit-ready paper claim.

For paper submission

Use approved red OCR form stock when required by the payer. The form background, dimensions, and print alignment are designed for claim-scanning systems. The NUCC also warns that its downloadable form image should not be used to create forms for claim submission.

Check the payer

Requirements can differ among Medicare, Medicaid programs, workers’ compensation programs, and commercial plans. Verify the accepted form version, mailing address, attachments, and any paper-filing exception before sending the claim.
