Do not copy stale charges
Fee schedules can change. A saved template should not silently preserve an old charge without review.

Charge review
- Verify that Item 24F contains the billed charge intended for that service line.
- Check the amount after changing procedure codes, modifiers, or units.
- Do not treat a prior allowed amount or patient balance as the default billed charge.
Keep billed charges separate from payment information
Item 24F is part of the submitted service line. It should not be populated from an allowed amount, payment amount, contractual adjustment, or patient balance. Those values arise later in the revenue-cycle process. If an office maintains a fee schedule, changes to procedure codes or units should trigger a fresh charge review. When a corrected claim changes a service line, verify that the charge still represents what the practice intends to bill for that revised line.
