ABOUT THE GOVERNMENT FORM
The eMedNY-150003 is New York Medicaid’s Medical Assistance Health Insurance Claim Form for professional paper claims. It is a New York-specific form and is not interchangeable with a standard CMS-1500. When a provider must submit an original paper claim to Medicaid, the form has to meet eMedNY’s requirements.
While most routine professional claims can be handled electronically through 837P or ePACES, New York State’s September 2026 Medicaid Update reiterates that certain practitioner-administered drugs designated By Report (BR), including unclassified drugs, continue to use eMedNY-150003 paper claims with manufacturer invoices and drug details. Providers must report the appropriate HCPCS code, National Drug Code (NDC) and quantity as applicable.
THE PRINTING PROBLEM
New York Medicaid requires original paper claim forms for paper submissions, not the downloadable sample PDF. The envisioned program would print only the claim data onto the state-issued preprinted eMedNY-150003 form, with printer-alignment controls to keep values inside the correct fields.
Instead of positioning text in a word processor or repeatedly retyping a form, the proposed utility would use a dedicated data-entry workspace and printable field layout. An alignment control would help compensate for different printers and paper feeds. For DC-217, California also recommends plain 10-point fonts, uppercase printing and six-digit dates without separators.
These are proposed capabilities, not currently shipping features. The program would be a focused paper-claim preparation tool rather than a full medical or dental practice-management system.
Organize member information, billing provider identifiers and service details in an editable form.
Prepare relevant professional claim information including dates, procedure codes, amounts and units.
Support fields and a checklist for HCPCS, NDC and quantity when a paper drug claim requires them.
Test horizontal and vertical alignment so black claim data lands on preprinted paper correctly.
Reuse provider details and reopen saved claim drafts to reduce repeated entry.
Remind staff to review required invoices, clinical worksheets or other attachments before mailing.
