Key points
- Confirm patient identification, agency information, start-of-care context, and the plan or certification period.
- Review diagnoses, skilled services, orders, frequencies, medications, functional information, goals, and other patient-specific elements against the chart.
- Check responsible practitioner information, required signatures, dates, face-to-face documentation, and current certification or recertification requirements.
First: confirm the recipient still wants a CMS-485-style document
CMS-485 is a legacy Home Health Certification and Plan of Care format. Before spending time perfecting the form layout, confirm that the agency, payer, referral source, or internal workflow actually expects this format. Current Medicare compliance turns on the required certification, plan-of-care, face-to-face, and supporting documentation—not on universal use of Form CMS-485 itself.
Second: verify identity and episode information
Check the patient record, agency information, episode or plan dates, diagnoses, and practitioner information against the current source documents. If the file began from a prior patient or a reusable template, explicitly clear claim-specific dates, diagnoses, orders, medications, goals, and certification information before entering the new plan.
Third: review the clinical plan as a connected document
Orders, frequencies, goals, diagnoses, functional information, and practitioner certification should make sense together and reflect the current plan of care. A complete-looking printout is not enough if the underlying clinical documentation or required signatures/dates are missing.
Finally: control the version that leaves the office
Preview for clipped or misaligned text, obtain the signatures or attestations required by the recipient, and retain the final version together with the supporting record. Record when and where it was sent so later questions can be tied to the exact document.
Legacy CMS-485 workflow check
- Confirm patient/episode identity first, then review diagnoses, orders, goals, treatments, and practitioner information.
- Check required signatures/dates and any supporting home-health documentation outside the form itself.
- Retain the final output together with the underlying plan-of-care record.
Sources and verification
Use current CMS home-health guidance to verify the underlying certification and plan-of-care requirements; confirm separately whether the recipient still wants a CMS-485-style document.
