Use enrollment data as the source
Billing-provider information is not simply contact information for the office. It is part of the payer-facing identity of the entity submitting the claim. Maintain a controlled billing-provider record containing the current legal/business name, address, NPI, and any payer-required identifiers. When enrollment information changes, update that source record and then review claims created from older templates so historical values do not continue printing in Item 33.
