Verify the address

Use a current payer source for paper-claim mailing addresses. Do not rely on an old claim or search result when the plan provides a current address.

Assemble attachments

Include only documents required for the claim and label them so they can be matched to the patient and services.

Protect the form

Use the office's normal mailing process so the form remains readable and patient information is handled appropriately.

Retain the package

Keep a copy of the claim and relevant attachments plus the date sent.

A consistent package order makes it easier for a second staff member to perform a quick quality check. Record the payer address source and mailing date with the claim so returned mail or timely-filing questions can be researched without reconstructing the package from memory.
